Non-Tobacco User Discount for Benefits


Celadon offers a discount applied to your medical premium if you are a non-tobacco user. To receive this discount, you MUST take a smokelyzer test to prove you do not use tobacco. The test is simple and takes only a few minutes. The test and non-tobacco user affidavit form must be turned in before 11:59 p.m. on Dec. 23, 2015. If you do not meet this requirement, you will pay the standard insurance premium beginning Jan. 1, 2016.

You may take the test at a primary physician or doctor’s office, but the Verve Clinic at Celadon headquarters offers these tests for free. You do not need an appointment to take the test at our Indianapolis location. Traveling nurses will visit outside terminals to complete tests as well. Look out for information on their schedule from terminal managers.

Current announced smokelyzer test locations:

  • Columbus, OH–Thursday, Dec. 17, 9 a.m.-12 p.m.
  • Butler, Garrett, and Warren–Thursday, Dec. 17, 9:30-10 a.m., 1:30-2:15 p.m., 3:15-4:15 p.m.

You must turn in both the test results and the non-tobacco user affidavit in order to receive the discount. The tobacco affidavit is a legal document you (and your spouse, if applicable) sign, stating you do not use tobacco products and have not for the past 12 months.

You must complete a spousal affidavit if you want to add your legally married spouse to your health insurance. To qualify, your spouse must be unemployed or not offered health insurance through their own employer. This document must also be turned in by Dec. 23.

      If I wanted to say quit tobacco use now how long must I wait to take the test to get the discount.

        To sign off on the affidavit, you have to not have smoked in the past 12 months. However, you can go through a program called 1-800-QUITNOW.
